"YES" to all of your questions. The "hole" in the flat edge of the fender is square to accept the square detail on the shank of the threaded hold down bolt/rod. The hole is in the side of the battery tray for the opposite hold down bolt/rod. The end opposite the threaded end of this hold down bolt/rod has a 90 degree angle detail on it. The threaded end of the bolt/rod is routed from under the battery tray thru the hole. Reference attached pictures.

1. The "hole" on yours is drilled down into the flat edge of the fender next to the battery tray. Not in the sidewall of the fender. Was wondering because one of the bolts on reproduction looks longer than the other. My 66/67 actual has a hole up in the sidewall of the fender not down in the flat edge.

2. The hole on yours on the fender side is offset so you need to use the 45° style hold down rather than a conventional straight one? That is the way your picture looks.

3. There are no holes in the top of the battery tray itself (other than to drain water)?
